Follow Up Visits · For all Services

Follow-up visits are scheduled as needed to review your progress, adjust your plan, and make sure your care stays aligned with your goals. If you request an additional visit outside the planned schedule, we will review whether it is medically appropriate and help determine the best next step. Not every request will require a separate visit, and any added appointments are arranged based on clinical need.

6-Month HRT/TRT Comprehensive Wellness Program

A personalized continued care program designed for patients undergoing Hormone Replacement Therapy (HRT) and/or Testosterone Replacement Therapy (TRT) who want ongoing support, monitoring, education, laboratory review, and treatment optimization.

Optional: Initial Medical Weight Loss Management/Longevity Consult if desired. *medications and monthly refill not included.
Initial baseline laboratory review appointment
All standard baseline and standard follow-up laboratory testing during the first 6 months
Four (4) follow-up appointments

Ongoing Monthly Care
For continuing HRT/TRT patients who have completed the initial 6-month treatment program, achieved relative treatment stability, and want continued medical support, monitoring, and prescription management for therapies such as BHRT, TRT, thyroid management, longevity therapies, and wellness and preventive optimization support.
HRT/TRT Maintenace Fee after 6 month program includes:

What your ongoing medical oversight includes:
Patients who engage medical weight loss or longevity services are entitled to one follow-up visit per month. Additional visits requested by patient are $150 each.

Treatment recommendations, prescriptions, supplements, and therapies are individualized and prescribed only when medically appropriate and medically necessary. Not all patients will qualify for treatment, prescriptions, or specific therapies.
Insurance & Payment
Core Function Health is a cash-based specialty wellness practice.
We do not accept health insurance.
We are not contracted with or participating in any insurance plans.
We do not bill insurance carriers.
This model allows for a more personalized and comprehensive approach to patient care without many of the limitations commonly associated with insurance-based medicine.
Prescribing & California Residency Policy
Patients must be physically located within the state of California at the time of their appointment, in accordance with California medical practice laws and regulations.
Prescriptions, supplements, longevity therapies, laboratory testing, and treatment recommendations are individualized and prescribed on a case-by-case basis when deemed medically appropriate and medically necessary by the provider.
Not all patients will qualify for treatment, prescriptions, or specific therapies.
